If you are a candidate on an eligible list, you must submit 3 forms of acceptable documentation with your request from the list below: - Valid Driver's License
- Property tax receipt
- Postmarked mail with candidate name
- Utility bill
- Lease agreement or mortgage statement
- Voter registration card
- College enrollment documents
- Bank, credit card statement, insurance policy or insurance bill
- Cross-filer (Other State & County Exams)
If you have applied for one or more examinations offered by the County of Rockland, New York State, or other County Civil Commissions on the same date, you must complete this form and file it at the County of Rockland, Department of Personnel, no later than two weeks prior to the exam date. - Alternate Test Date Policy including Request for a Religious Accommodation

If you have received notification that you were removed from an eligible list, you may request to be reinstated to the eligible list by completing the attached form. Your request to be reinstated must include an explanation of the circumstances that resulted in your removal from the list for reasons such as an affirmative declination to the position on a prior canvass, failure to reply to a canvass, failure to appear for an interview or to accept appointment or to report to work. Supporting documentation may be attached as well. Pursuant to Rockland Count Civil Service Rule XI.3, all requests to be reinstated are made at the discretion of the Commissioner of Personnel
