Adult Immunization Clinic

Provides immunizations (shots, vaccines) for adults to prevent a variety of illnesses.

Better Choices, Better Health: Managing Ongoing Health Conditions

Can help you learn how to better manage your long-term health problems and lead an active, full life.  

Cancer Awareness & Education Program 

Provides information about reducing your risk of cancer, and referrals to local services, including free cancer screening programs.

Chronic Disease Prevention Workshops

  • Living Well with Diabetes Program can help you learn simple ways to take good care of yourself and manage your type 2 diabetes. Call us at (845) 364-2500 to find out when our next program is scheduled to begin.

  • Diabetes Prevention Program can help you prevent type 2 diabetes by learning how to: improve your food choices and lose weight; increase your physical activity level; and learn coping skills to maintain weight loss.  Are you at risk for diabetes? Fall Prevention

Learn tips to reduce your risk of falling. 

Flu Clinic

Provides flu vaccines to people ages 6 months and older, and is usually held September - January.

HIV/AIDS Services

This provides rapid testing for HIV (results in 20 minutes). Medical and case management services are provided for people over the age of 18 living with HIV/AIDS.

Home Health Care Program

Provides home visits by nurses to assess the health status of a patient, offer health guidance, or evaluate placement in a facility.

Immunization Program

We offer many clinics where residents of all ages can get the immunizations (shots, vaccines) you need to protect yourself and others from serious diseases and infections. We also can help you find reliable information on immunizations.

Infectious Disease (ID) Clinic

Provides medical services, nutrition counseling, and case management services to people over the age of 18 who are living with HIV/AIDS.


Looking for information about good nutrition, healthier cooking, weight loss, or where to find better food choices in your community? Then you've come to the right place! We can help you (and your family, friends and co-workers) find, prepare, and share healthier food (and beverage) choices.

Quit Smoking Program

Our award winning, highly successful Put It Out Rockland program offers a series of one-on-one, or group, quit smoking sessions with a quit smoking specialist and low-cost nicotine replacement products (for those medically eligible) to double your chances of being successful. Call 364-2651 to get help to quit.

Sexually Transmitted Infections Clinic

Will provide confidential testing and treatment for a variety of sexually transmitted infections, and free HIV tests. Please note: beginning January 2018 the Sexually Transmitted Infection Clinic (STI) will be renamed Sexual Health Clinic.

Tuberculosis (TB) Services

Includes a chest clinic for evaluation and treatment of people with active TB, their contacts and other people at high risk for TB, and a clinic that performs tuberculin skin tests to check for TB infection.

Walking Programs

Engaging in a walking program can help reduce the risk of heart disease, burn calories, lower blood pressure, increase muscle tone, improve circulation, prevent bone loss, and make you feel good. Our 'Rockland County Walks' Initiative consists of several walking opportunities.

Weight Loss

Ready to lose weight? Join the Lose to Win Weight Loss Program - an eight-week series that uses the latest guidelines to help you lose weight safely and successfully.

Worksite Wellness Program

Provides free wellness assessments, resources, presentations, and other services to assist Rockland employers in creating healthier workplace environments.
