Adult Day Care - Social Model

Adult Day Care - Medical Model
- Friedwald Center's Adult Day Health Care Program
475 New Hempstead Road, New City, NY - Provides rehabilitation therapy and social work services. The staff administers medication, provides meals and personal care assistance. (845) 678-2100 - Northern Metropolitan Adult Day Health Care Center
225 Maple Avenue, Monsey, NY - Provides nursing, case management, medical social worker, nutritional and recreational services. A specialized services program is available for people with dementia. (845) 352-9000
Aging Support Group
Support groups designed to alleviate stress or anxiety and to help the clients make appropriate choices and plans. The goal is to assist older persons in coping with the problems and stresses which interfere with normal health and social functioning. Alzheimer's Disease/Dementia Support

Provides for the rapid dissemination of information regarding missing seniors who are suffering from Alzheimer's disease, dementia or other cognitive or intellectual impairments which place them at risk.

Caregiver Support Group

Caregiver Respite
The Rockland County Office for the Aging Respite Program is a caregiver program that pays for a free week-long stay at an assisted living facility. A new home care component to the program allows an individual to remain in their home with an aide for a specific period of time. This respite program allows for a break in the daily routine for both the adult providing assistance to an older person and for the older person him/herself. This program is available for individuals 60 and older that reside in Rockland County. The Office for the Aging has been able to utilize some of the funding available under Title IIIE of the Older Americans Act to create this program to give needed support to caregivers. The costs are completely paid by the caregiver program. The program does ask for an evaluation of the experience and affords the participant an opportunity to make a voluntary contribution.

Geriatric Care Management
Program aimed at developing and implementing care plans for vulnerable seniors.

Mental Health Association of Rockland County
Provides mental health preventative, supportive, treatment and advocacy services to the community. Includes comprehensive list of programs. (845) 267-2172
Helps family caregivers and health care providers work together to plan and implement safe and smooth transitions between care settings for chronically or seriously ill seniors.
Older Adult Counseling Program
One-on-one program to support older adults who are isolated because of physical and /or cognitive limitations and would benefit from discussing age-related concerns with professionally trained volunteers.

Parkinson's Support Groups
Support group geared toward individuals who have been recently diagnosed with Parkinson's disease. The group meets on the fourth Sunday of each month from 10:00 am - 11:00 am at Rockland Jewish Family Service, located at 450 West Nyack Road, Suite 2, West Nyack, NY 10994. Weekly meetings - one hour of exercise by a physical therapist from Nyack Hospital. Caregivers have an opportunity to share, discuss issues and receive assistance and support from others in a comfortable environment. Call Joan James at (845) 358-3128 for more information.
United Hospice of Rockland, Inc.
Provides care, hope, comfort and improved quality of life to individuals and their families facing a serious illness. (845) 634-4974
Contributions are voluntary and confidential and are used to expand the services for which they are received. No one will be denied services because of an inability or unwillingness to contribute.
